Next Generation NCLEX and Clinical Judgement

by Kendra S. Lindloff, MSN, RN, CNE

The Next Generation NCLEX (NGN) will launch in April 1, 2023. The National Council of State Boards of Nursing (NCSBN) research and practice analysis over several years has revealed the need to create current, and valid NCLEX items to evaluate clinical judgement of nursing program graduates. As a result of this research the NCSBN Clinical Judgment Measurement Model (NCJMM) was developed. This model incorporates important elements of the nursing process, client factors, environmental factors, and clinical judgment. Clinical judgement provides the candidate a framework to working through client problem. These steps could include the following: recognizing cues, analyzing cues, prioritizing hypothesis, generating a solution, taking action, and evaluating outcomes.

The NGN will incorporate item types that are case studies to mimic clinical scenarios. These new item types do not replace the traditional items but serve as a way to assess the candidates understanding and clinical judgment to entry-level nursing knowledge. This will allow the candidate an opportunity to demonstrate the knowledge, skills, and abilities learned within their nursing program. Another advantage of NGN item types is the ability to have polytomous scoring or partial credit of a question.  The intent is to validate a graduates knowledge so they can practice safely, competently and achieve the best outcomes for clients, nurses, and institutions.

Educators have had the opportunity and challenge to incorporate clinical judgment in didactic, simulation, lab, clinical, and preceptorship experiences for student. The NCSBN has many resources to support the implementation of NGN such as webinars, quarterly newsletters, NGN talks and videos. In addition there is a section of Frequently Asked Questions (FAQ's) that addresses questions from candidates and educators.

The NGN exam will remain similar to the current NCLEX with number of items, exam length, computer adaptive testing (CAT) methodology, and stopping rules to determine pass/fail status. These practices ensure that candidates receive a balanced assessment of their knowledge regardless of ability. For those candidates that need testing accommodations there have been no changes to that process. Please see the Iowa Board of Nursing website for the request for testing accommodations.
