All dental hygiene licenses, local anesthesia permits, dental assistant registrations and radiography qualifications will expire on August 31, 2019. Renewal notifications will be sent in late June. In order to ensure receipt of this and other notifications, please review the following information.
Iowa Administrative Code 650 – 10.6(2) requires licensees and registrants to notify the Board in writing of changes in email and mailing/work addresses within 60 days. In order to ensure receipt of correspondence from the Iowa Dental Board, please update the following information if applicable.
- Email address: Consider using a personal email address so that changes in employment do not cause disruptions in the receipt of communications from the Board.
- Primary mailing address: You may designate a home or work location as your primary mailing address.
- Practice locations: Report all full-time or part-time practice locations.

NAME CHANGES
Iowa Administrative Code 650 – 10.6(1) requires licensees and registrants to notify the Board of all legal name changes within 60 days. You may submit the proof of legal name change via email, fax or mail to the Board’s office.
Do not submit documentation of a name change with your renewal. Online renewal is automated. In order to ensure that the correct name prints on your renewal card, please submit name changes a minimum of 10 business days prior to submitting an application for renewal.
