We are excited to see the expansion of Person-Centered Service across our region, and very happy to announce that ConnectWellSD is expanding partnerships with 2-1-1 San Diego and CIE San Diego to broaden the reach of point-to-point referrals created in the system.
Community Information Exchange (CIE) San Diego is a network of social service and healthcare providers sharing a technology platform that fosters information-sharing and deeper collaboration.
What Does This Mean for ConnectWellSD?
It means that more community providers can receive electronic referrals in ConnectWellSD than ever before! Previously, only two community providers were in the system. Today there are 70+ providers -- and more than 200 services!
To keep you informed, we'll be sending messages highlighting services from featured providers.
For starters, we'd like to feature two service providers offering grocery delivery to eligible customers who are isolated by COVID-19: the City of Chula Vista and FACT.
