Vaccine Survey

County of Los Angeles, California sent this bulletin at 11/27/2023 11:32 AM PST

Dear Los Angeles County Skilled Nursing Facilities,

We wanted to let you know that after Thanksgiving, Los Angeles County Department of Public Health will be sending a short survey to all healthcare facilities covered under Los Angeles County Health Officer Order 2023-04-01 to assess Influenza and COVID-19 vaccination rates among healthcare personnel. This is a required survey under Section (8) of the Health Officer Order.

Specific information that will be required includes:

Total number of healthcare personnel that were eligible to have worked at least 1 day during the week of data collection November 27, 2023 through December 3, 2023
Number of personnel vaccinated with both the 2023-2024 updated COVID-19 vaccine and influenza vaccine
Number of personnel that have been vaccinated with the influenza vaccine but not the 2023-2024 updated COVID-19 vaccine
Number of personnel that have been vaccinated with the 2023-2024 updated COVID-19 vaccines but not the influenza vaccine
Number of personnel that have not been vaccinated with either the 2023-2024 updated COVID-19 vaccine and influenza vaccine
If your healthcare facility has a policy to require masking in patient care areas if healthcare workers are unvaccinated as required by the Health Officer Order.

This survey is required for the following healthcare facilities covered under the order:

Intermediate Care Facilities
Emergency Medical Services (EMS) Provider Agencies
Congregate Living Healthcare Facilities
Chemical Dependency Recovery Hospitals
Acute Psychiatric Hospitals
Home Health Agencies
Primary Care Clinics
Ambulatory Surgery Centers
Hospice Facilities

In lieu of the survey, General Acute Care Hospitals, Long-Term Acute Care Hospitals, and Skilled Nursing Facilities should report vaccination coverage for the week of November 27, 2023 through December 3, 2023 in the National Healthcare Safety Network (NHSN) Weekly Flu HCP Vaccination Summary and Weekly COVID-19 Vaccination Module for Healthcare Personnel. Dialysis Centers should report vaccination coverage for the week of November 20, 2023 through November 26, 2023 in the National Healthcare Safety Network (NHSN) Weekly Influenza Reporting Module and Healthcare Personnel COVID-19 Vaccination module.

The survey link will be forwarded to healthcare facilities for completion the week of December 4.
