Dear Skilled Nursing Facility partners in Los Angeles County,
The Los Angeles County (LAC) Department of Public Health (DPH) is excited to announce applications are opening very soon for the 2023-2024 Updated COVID-19 Financial Reward Program! This program is only available to skilled nursing facilities (SNFs) in LAC including Pasadena and Long Beach. Please click on this link (attachment) for details on the two rewards that are offered this year to the first 100 facilities to or until funds run out - $7,500 per facility for having at least 80% of residents up to date with the 2023-2024 formulation of the COVID-19 vaccine and/or $2,500 per facility for having at least 80% of staff up to date. The resident and staff census must include everyone who resided or worked at the facility at any time over a 1-week period from Monday through Sunday between Nov 13 thru Dec 17, 2023. If facilities qualify for both rewards, they could be rewarded $10,000 in total.

In preparation for applications opening NEXT MONDAY, Nov 20, 2023, we are providing a brief overview of the application process:
- Submit an application. An application must contain both of the following AND must be submitted between Nov 20 and Dec 22, 2023.
- Online application form (link coming soon)
- Person-level vaccination information for your entire resident and/or staff census over the 1-week period when you believed your facility achieved at least 80%. The 1-week period must be Monday thru Sunday anytime between Nov 13 thru Dec 17, 2023. (more details coming soon)
- Your facility’s application is only complete if you receive a confirmation from COVID-LTC-Test@ph.lacounty.gov.
- Continue to keep a close eye on your email in case our team requests for more information or a re-submission.
