  Attention DHS Workforce:
DHS is now accepting requests for accommodations and exemptions for the vaccine mandates.
As a result of the County’s Vaccine Mandate and the California Dept. of Public Health Order, dated August 3, 2021, all DHS employees are required to be fully vaccinated, unless granted an exemption. DHS employees may request an exemption from the County’s Policy and the CDPH order on the following grounds:
- A medical condition or disability that does not allow them to get vaccinated against COVID-19; or
- A sincerely held religious belief, practice, or observance that prohibits the employee from receiving a COVID-19 vaccine. (NOTE: social, political, or economic philosophies, as well as personal preferences, are not sufficient justification for granting an exemption.
DHS has set up a central process for review of these requests. If you plan to submit an exemption request, please do not submit your request to your supervisor or facility. Please see below for instruction on submitting an exemption form:
Step 1: Fill out exemption form. Form to request an exemption can be found below. Please make sure to submit all requested documentation, i.e. Healthcare Provider Certification.
Step 2: Submit your form to the appropriate INBOX for your request.
Submitting your form to the appropriate inbox in help ensure proper review and the confidentiality of any medical information provided.
You will receive confirmation that your form was received within 3 business days of your request. If DHS needs further information about your request, you will be contacted by an analyst for a telephonic Interactive Process Meeting.
Within 30 business days, you will be notified of DHS’ determination on your request for accommodation and/or exemption. If your request is denied, you will have the opportunity to seek reconsideration from the LA County’s Department of Human Resources.
Finally, employees submitting a request for vaccine accommodation/exemption will be required to get a weekly or bi-weekly COVID-19 test before and after the accommodation is approved. DHS workforce does NOT need to verify their vaccination status at fulgent genetics. If you are not vaccinated or have not submitted proof of vaccination to DHS’ Employee Health Services, we already have your vaccination status verified. Staff who are not vaccinated will receive notification of the weekly or bi-weekly testing requirement.
