Dear DHS Workforce,
LA County recently sent the email copied below to ALL LA County employees. The email contains directions on how to submit vaccine proof as part of your need to meet the LA County Executive Order requiring ALL LA County employees to be vaccinated or submit proof of vaccination by October 1, 2021.
Since DHS already vaccinated over 75% of its employees, our DHS workforce does NOT need to submit proof of vaccination to Fulgent. We have our own process for submitting proof of vaccination.
- If you received your vaccine via DHS – you do not need to do anything, we already have your record!
- If you received your vaccine via your provider or any vaccine site outside of DHS please follow the directions below to submit proof of your vaccine as soon as possible.
Submit Your Proof of Vaccination
Employee Health Services is collecting proof of vaccination for all employees and contractors who received vaccine outside of DHS. Please note that contractors are still required to submit proof directly to DHS, even if they have already provided to their sponsoring organization.
Steps
- Make a pdf or jpg copy of your vaccine card
- Email the image to your Employee Health representative (see email addresses below)
Or drop off a copy in person at your local Employee Health Services.
