State of California Digital
COVID-19 Vaccine Record Portal

The California Department of Public Health (CDPH) has launched a Digital COVID-19 Vaccine Record portal, which Californians can use to get a digital copy of their vaccine records. This online portal is optional and not a vaccine passport. The online record will have the same information as the paper CDC vaccine card and you are the only one who can decide when and how to share your vaccine record.

For more information, visit the CDPH Digital COVID-19 Vaccine Record FAQs page.
