Order Immunization Resources with a New Online Form!
 Healthcare Team Members & School Staff,
We have implemented an online order system to allow for easier management of request orders from the Bureau of Immunization Services (BIZS). BIZS forms that are available to order include the Arizona Lifetime Immunization Record Cards (Blue Books) and School Immunization Record Forms.

Visit the BIZS website to access the order form and other BIZS resources.
What does this mean to you:
- This online order form replaces the previous pdf order form. No more need to download or print the pdf request form and send via fax or email.
- We will no longer accept the previous pdf order form submissions.
- Once your order is submitted, you will receive a confirmation email with a link to check the status of your order.
- Turnaround time for orders will remain the same.
- This new system allows for better communication from our office to yours.
