What Arizona VFC Providers Need to Know
The 2021-2022 Influenza Season is coming to an end and all influenza vaccines will expire on 06/30/2022.
If your facility has any influenza vaccine in a cold storage unit please follow the following steps when the vaccine expires at the end of the day:
- Remove all wasted/expired vaccines from the cold storage unit(s)
- Login to ASIIS
- Ensure all administered influenza vaccine were properly decremented from the inventory and linked to the the patient record
- In the menu on the left side of the screen, click on “Lot Numbers” and then click on Reconciliation.
- Find each lot number to be reconciled
- In the Physical Inventory column, enter 0 doses as the total.
- Enter “Expired” as the Category from the drop-down list.
- Enter “Expired” or “Expired multidose vial” as the Reason from the drop down list.
- Check the Inactive box on the far right and click on Save at the bottom of the screen.
- Repeat steps 5-9 for each expired lot number.
- Fill out and sign the Wasted/Expired Vaccine Return Form
- Email the completed wasted/expired vaccine return form to arizonavfc@azdhs.gov or fax to (602) 364-3276 or (602) 364-3232.
- Place the expired vaccines in a labeled DO NOT USE box.
- Once your return is processed an email will be sent from the AIPO with a copy of your processed return receipt with further instructions.
- Place a copy of the return receipt in the box with the vaccines
- McKesson will send the return label to the contact listed on the wasted/expired vaccine return form. Attach the label(s) on the appropriate box or boxes.
- Your regular UPS driver should be given the packed vaccines. If you do not have a regular UPS driver you can bring the package to your closest UPS store.
