Today’s Topic: Requests for COVID-19 Exemptions
The Arizona Department of Health Services (ADHS) does not have a COVID-19 vaccination exemption form available for adults.
The Arizona state rules and statutes on immunization requirements and exemptions are only in place for children in child care, preschool and kindergarten through 12th grades (K-12). These rules and statutes do NOT apply to adults. Additionally, the COVID-19 vaccine is not a required vaccine for Arizona schools, and is not included on any of the ADHS school or child care exemption forms.
Individual facilities and employers (college/university, hospital, medical clinic, business, etc.) may choose to set their own immunization requirements for adults (employees or otherwise). Individual facilities and employers could consider allowing for religious or medical exemptions.
Adults who have questions on immunization exemptions, for any vaccine, should contact the facility or employer who has set the requirement. ADHS cannot and will not provide exemption forms for adults, regardless of the existence of individual facility or employer requirements.
