 Provider Agency Department Approval Renewal Reminders

The purpose of this e-memo it to remind providers of requirements to maintain and extend Department Approvals and Enrollments per 7 AAC 70.010 – 150 and 7 AAC 105.210.  Providers must seek renewal of a Department Approval before the expiration date.

  • Prior to the expiration of the approval, contact the MPASS inbox at the email address below and include the following:


  • Provider number(s) including confirmation of the physical address and services conducted as listed on the application forms
  • Current approval and latest report from an accreditation agency (COA, CARF, TJC) or any correspondence from them that it is delayed and in process.
  • If your agency or any of its locations received 1115 SUD approval prior to the revised application form listing out services, fill out and re-submit selecting services conducted and billed. Link to applications:

Please also note, for those agencies that already have Department approval for any of the 1115 services outlined in the Provider Checklist Level of Care Service Component Review e-memo dated 6/21/2021 and have not submitted program documentation, please do so immediately.  Once reviewed and approved, the agency will move from a provisional 1115 approval to a full Departmental approval.
