SDS has updated the three Frequently Asked Questions documents and the Instructions for Completing the COVID-19 Appendix K Person-Centered Support Plan Request on the SDS COVID page.  

 

Regarding Annual Level of Care Applications:

 

SDS is extending for one year level of care (LOC) determinations that expire before 12/31/20, so care coordinators do not need to submit a Uni-04 for these recipients.   Care coordinators do need to submit a support plan renewal, either on the COVID-19 Appendix K Support Plan Request Form (see below) or the Uni-02, if changes to the support plan are requested beyond what the COVID-19 Appendix K Support Plan Request Form can be used for.

 

For LOC expiring after 12/31/2020, care coordinators need to submit the complete application packet for all waiver types no later than 90 days ahead of LOC expiration, per 7 AAC 130.213. Assessment and reassessment (c).   Care coordinators should have LOC documents (VOD, QDC, ICAP materials, medical documentation) by about Oct 1 to submit for applications expiring shortly after 12/31/20.

Regarding the COVID-19 Appendix K Support Plan Request Form:

 

While the COVID-19 emergency declaration remains in effect, care coordinators should use the COVID-19 Appendix K Support Plan Request form to renew an annual plan with a start date of 12/31/2020 or sooner, as long as the plan has no changes in service, provider, and/or service amount requested.

 

Follow regulation for timely submission of plans: 7 AAC 130.217. Plan of care development and amendment (5) (B) 30 days before the expiration of the current plan year.

 

Please note:

 

To assist in timely processing, SDS will accept the annual support plan renewal up to 45 days ahead of the expiration of the current plan year.

• Do not submit the Appendix K Support Plan Request form earlier than 45 days ahead of the plan renewal date. Early submissions will be returned to the care coordinator.    

• Do not use the Appendix K Support Plan Request form to submit annual plans with changes in service, provider, or amounts outside of the COVID-19 related needs on the form.

• If the COVID-19 emergency declaration ends before 12/31/2020, SDS will notify care coordinators of revisions to the above.
