Dear Fairbanks and Juneau providers:

Onsite Reviews by the Department of Health and Social Services is necessary to move forward on your 1115 waiver facility enrollment with Conduent. Please sign up for EACH of your facilities to have an onsite review in the Fairbanks and Juneau area locations in the evite invitation below. You MUST have an 1115 waiver facility Department Approval certificate before this onsite can be conducted. Please have a few personnel records available for each location.

You cannot finish enrollment in Conduent unless this onsite is conducted.
