As part of the Wisconsin Division of Public Health (DPH) investigation of Elizabethkingia bloodstream infections occurring during November 2015 to the present, we are conducting a survey among Wisconsin assisted living facilities and home health agencies to identify brands and distributors of selected products used among your residents or clients, and among your healthcare personnel.
In conducting this survey, DPH is not implicating any product, manufacturer, distributor or healthcare organization regarding the outbreak, rather the survey will help focus our efforts to identify the source of the Elizabethkingia infections. All responses will be kept confidential.
