The state prototype Free and Reduced-Price Meal Application for school year 2020-21 will be posted on the NSLP Free and Reduced-Price webpage, under the 'Application Materials' dropdown.
The Free and Reduced-Price Application is available as a Word Document or as a fillable PDF.
Local education agencies (LEAs) may use the state prototype meal application materials or the United States Department of Agriculture (USDA) meal application materials. LEAs may not produce their own materials or alter the state prototype materials.
Reminder
Public School Districts must include the district non-discrimination statement on the free and reduced-price meal application. Public school district civil rights statements must be included on widely disseminated information, including the free and reduced-price meal application. This was a change implemented in SY19-20. To accommodate this requirement, the “other benefits” box was removed from page 2 of the state prototype meal application. This allows space for the district civil rights statement.
Questions about these requirements? Please contact your district Civil Rights team or the OSPI Equity and Civil Rights team.
Consent for Other Benefits
LEAs that gather parental consent for local programs that offer reduced fees for students eligible for meal benefits can use the Consent to Share Eligibility Form. This form is also available in Spanish.
