The DSHS Aging and Long-Term Support Administration launched CV19 CheckUp. This new tool is free, confidential, and easy to use. It is designed to help you be safer and healthier during the COVID-19 pandemic. 

After completing the questionnaire, the CV19 CheckUp immediately delivers a personalized report answering questions such as:

• How likely am I to get COVID-19?
• How likely am I to spread COVID-19 to others if I contract it?
• If I get COVID-19, how severe would it likely be?
• What steps can I take to reduce my risks of getting COVID-19?
• How can I find help to make sure my needs are met during the pandemic?

Although there isn't enough vaccine for everyone at this time, you can find out if you are eligible for a COVID-19 vaccine by using the Washington State Department of Health's tool, http://findyourphasewa.org/. This new tool is also free, confidential, and easy to use.

If you are not eligible yet, you can provide your contact information and the Washington State Department of Health can notify you when you become eligible. You can also check back later.

If you are eligible, you can print or use the screen shot of the confirmation page from Phase Finder. You should be prepared to share that confirmation along with an identification card to the vaccine provider as proof of eligibility.
