Vaccination Process for Companion Home (CH) and Alternative Living (AL) Providers.

The Washington State Department of Health (DOH) has included Long-Term Care Workers, which includes CH and AL providers, in Phase 1a of the COVID-19 vaccination distribution plan.

CH and AL providers who offer face-to-face care or personal care tasks in client homes may wish to complete the COVID-19 vaccination series. To obtain the vaccination, the CH or AL provider must provide one of the following verification documents to the vaccine provider.

Verification Option 1

1.    Complete the PhaseFinder survey on DOH’s website.

2.    Screenshot or print results and give to the vaccine provider for verification.

Verification Option 2

1.    Receive a verification letter from the Department of Social and Health Services.

2.    Follow instructions in the verification letter on how to access the vaccine.

Vaccine Provider Locations

CH and AL providers can visit the DOH Vaccine Locations website to identify vaccine providers in their county. The website provides a list of locations, but the CH or AL provider will still need to contact a location to schedule an appointment.

Vaccines remain limited in number but will increase in the coming weeks. It is possible CH and AL providers will need to contact multiple vaccine providers to access the vaccine.
