NDEP’s Practice Transformation Online Resource Provides Practical Information and Tools for Physicians & Health Care Teams
A population approach to healthcare aims to improve the health of everyone in a clinical practice. This means it’s important to have a system where you know who your patients are and can evaluate patient data regularly to ensure that they are getting the care they need. Organized around the principles of the patient-centered medical home, NDEP’s Practice Transformation for Physicians and Health Care Teams resource provides models, links, and tools to help physicians, health care professionals, teams, and administrators improve their practice and their systems of health care delivery.

Pharmacy, Podiatry, Optometry, and Dentistry (PPOD) Toolkit and Webinars 
Too many people with diabetes are not getting their annual foot, eye and oral exams, or managing their medication. The NDEP’s enhanced Working Together to Manage Diabetes Toolkit empowers pharmacists, podiatrists, optometrists, dentists, and other providers to deliver more integrated care for patients with, or at risk for, diabetes. When you work in tandem with other health care providers, you become more effective at recognizing the early danger signs of diabetes, decreasing the risk of complications, and managing medication therapy. 

Register: Register for one of three NDEP-sponsored webinars to learn from PPOD professionals who have worked collaboratively in their communities to better care for patients at risk for diabetes and those who already have diabetes using PPOD tools.

2014 National Diabetes Statistics Report Released
The National Diabetes Statistics Report, 2014 presents the latest data on the burden of diabetes in the United States. Highlights of the report include:

  • 29.1 million people or 9.3% of the population have diabetes, up from 26 million in 2010.
  • 21.0 million people have been diagnosed with diabetes.
  • 8.1 million people are undiagnosed (27.8% of people with diabetes are undiagnosed).
