A new version of the CBP Form 5106 with an expiration date of 05/31/2016 has been posted at
http://www.cbp.gov/sites/default/files/documents/CBP%20Form%205106%20%2805-13%29.pdf
Although the new version can be used immediately, Office of Administration, Bond Team will grant a 30 day grace period to transition from the old version to the new version. Effective July 1, 2014 the new version of the CBP Form 5106 will be mandatory, and any other version will not be accepted.
