The summaries listed below have been withdrawn from the NQMC Web site because they are no longer in current use and therefore, do not meet the NQMC Inclusion Criteria. Please see the Measure Archive for a complete list of all measures that have been withdrawn from the NQMC Web site.

Physician Consortium for Performance Improvement®

  • Endoscopy and polyp surveillance: percentage of final colonoscopy reports for patients aged 18 years and older that include documentation of all of the following: pre-procedure risk assessment; depth of insertion; quality of the bowel prep; complete description of polyp(s) found, including location of each polyp, size, number and gross morphology; and recommendations for follow-up.
  • Hepatitis C: percentage of female patients aged 18 to 44 years and all men aged 18 years and older with a diagnosis of chronic hepatitis C who are receiving antiviral treatment who were counseled regarding contraception prior to the initiation of antiviral treatment.
  • Hepatitis C: percentage of patients aged 18 years and older with a diagnosis of chronic hepatitis C who were prescribed peginterferon and ribavirin therapy within the 12 month reporting period.
  • Hepatitis C: percentage of patients aged 18 years and older with a diagnosis of chronic hepatitis C with genotype 1 or 4 who are receiving antiviral treatment at week 24 for whom quantitative HCV RNA testing was performed at 24 weeks of treatment.
  • Osteoarthritis: percentage of patient visits for patients aged 21 years and older with a diagnosis of osteoarthritis during which an anti-inflammatory agent or analgesic was considered.
  • Osteoarthritis: percentage of patient visits for patients aged 21 years and older with a diagnosis of osteoarthritis during which gastrointestinal (GI) prophylaxis was considered.
  • Osteoarthritis: percentage of patient visits for patients aged 21 years and older with a diagnosis of osteoarthritis of the hip or knee during which therapeutic exercise for the hip or knee (therapeutic exercise instructed or physical therapy prescribed) was considered.
  • Osteoarthritis: percentage of patient visits for patients aged 21 years and older with a diagnosis of osteoarthritis with an assessment for use of anti-inflammatory or analgesic over-the-counter (OTC) medications.
  • Osteoarthritis: percentage of patients aged 21 years and older with a diagnosis of osteoarthritis for whom a physical examination of the involved joint was performed during the initial visit.
  • Osteoarthritis: percentage of patients aged 21 years and older with a diagnosis of osteoarthritis on prescribed or over-the-counter (OTC) non-steroidal anti-inflammatory drugs (NSAIDs) who were assessed for gastrointestinal (GI) and renal risk factors.
  • Preventive care and screening: percentage of female patients aged 21 through 65 years who have documentation of the performance of current cervical cancer screening with results during the two-year measurement period.
  • Preventive care and screening: percentage of female patients who had a mammogram performed during the two-year measurement period.
  • Preventive care and screening: percentage of patients aged 18 years and older for whom body mass index (BMI) is documented at least once during the two-year measurement period.
  • Preventive care and screening: percentage of patients aged 65 years and older who have documentation of receiving pneumococcal immunization during the two-year measurement period.
  • Preventive care and screening: percentage of patients screened for colorectal cancer during the one-year measurement period.

Notice to Our Users - Enhanced HHS Measures Inventory Coming Soon!

The HHS Measures Inventory is currently undergoing improvements to enhance the content, functionality, and overall user experience of the Web site.

  • Streamlined Measure Updating. The HHS Measures Inventory will transition from an annual to a "real-time" updating of content directly to the Web site.
  • Links to Expanded Measure Information. When available, external links will be established among measures in the HHS Measures Inventory and the same or similar measure residing in the National Quality Measures Clearinghouse (NQMC), the National Quality Forum (NQF), and the United States Health Information Knowledgebase (USHIK) repositories.
  • New Measure History Display. A complete history of entry actions, including measure submission, review, and the date and time of each action will be displayed for each measure record.
