Carers of People with a Learning
Disability - Have Your Say
Please note:A
carer is a family member or a friend who provides unpaid practical and/or
emotional support to someone in need of care.
Nottingham
City Council and Nottingham Clinical Commissioning Group are currently in the
process of reviewing services available to citizens with Learning Disabilities.
As
part of this we would like to find out the views of carers on a range of
subjects including housing, day and evening opportunities, health and carers
assessments. As such we are asking carers of people with a learning disability
to complete a survey.
The
survey can either be completed online at www.nottingham.gov.uk/LDcarershys, or via paper version. The surveycan
be returned to Glenys Townsend, Commissioning & Insight, Third Floor,
Loxley House, Station Street, Nottingham, NG2 3NG. A freepost envelope is
available if required, and can be obtained by contacting Glenys Townsend on
0115 8764833.We require all forms to be
completed and returned by 27th
March 2015.
The
survey should take around 10 minutes to complete and if contact details are
given all completed forms will be entered into a prize drawer for a £30
Victoria Centre Shopping voucher.
We
would love to hear from you, your views will help shape the services that we
provide and the way we provide them.

Monday

Glossary of terms : In order to help you to complete the survey, we have
included a glossary of terms below to explain any words or terms you may not be
familiar with.
Supported Living -Supported living means that you
are supported to live in the way you want, your housing and support is built
around you rather than you fitting into a service. You choose who you live with
(if anyone), you choose where you live, you choose who supports you and how you
are supported.
Shared Lives -Shared
Lives offers accommodation and support within approved family homes to people.
Extra Care - Extra Care extends the basic sheltered
housing concept. Typically there will be more extensive communal facilities
including a lounge, activities room, IT suite, shop, provision for at least
some meals, and 24 hour care available on site through a team of carers. Extra
Care schemes are based on a collection of self-contained dwellings but can be
much bigger extending to Extra Care villages of up to 300 properties.
Direct Payments- A direct payment is money
that is paid directly to you so you can arrange your own support.
