Contract Type: Permanent – Part Time

To create public confidence, public value and deliver excellent outcomes for older and disabled citizens and their carers through the leadership of and effective delivery of assessment, care management, review and safeguarding services. The Senior Practitioner will work alongside the Team Manager being responsible for a locality based Multi–disciplinary Team delivering social care services for older and physically disabled people and their careers.

The post-holder will carry an appropriate caseload

ESSENTIAL REQUIREMENTS:

• Appropriate professional qualification e.g. CQSW, DipSW, CSS, OT or Nursing degree.

• Thorough understanding of assessment processes and ability to assess highly complex needs and situations.

• Substantial knowledge as a social work practitioner with disabled people, carers and families across a range of functions.

• Extensive knowledge and experience of complex safeguarding work.

We are currently looking to recruit an Occupational Therapist to provide maternity cover for our Reablement Team. 

Job Purpose:

To carry out assessments in relation to disability and eligibility within current legislation, the City Council’s policies and core values, and Departmental policies and procedures. To develop person centred packages of support, which may include rehabilitation/reablement, provision of equipment and structural adaptations.

ESSENTIAL REQUIREMENTS:

- A Diploma / Degree in Occupational Therapy

- To be registered with Health Professions Council

- Ability to carry out community care assessments of need and develop care packages

- Awareness of equipment and adaptations in relation to promoting independence

Enhanced DBS Check with Children & Adults Barring List Check (regulated activity with Children & Adults).

This role will involve traveling to various parts of the city.

We are currently looking to recruit a qualified Occupational Therapist to join our Telecare Team.  

Job Purpose:

• To carry out assessments in relation to disability and eligibility within current legislation, the City Council’s policies and core values, and Departmental policies and procedures.

• To develop person centred packages of support, which may include rehabilitation/reablement, provision of equipment and structural adaptations

• As a Level 2 Occupational Therapist, you will carry a full caseload, including multifaceted cases and more complex manual handling assessments, and will work with minimum supervision.

• Knowledge of Telecare Equipment/Assistive Technology would be an advantage to this role, although not essential.

ESSENTIAL REQUIREMENTS:

•A Diploma / Degree in Occupational Therapy

•To be registered with Health Professions Council

•Significant relevant experience of working within Occupational Therapy

•Ability to carry out more complex community care assessments of need and develop care packages

Enhanced DBS Check with Children & Adults Barring List Check (regulated activity with Children & Adults)
