Fixed Term for 6 months plus Working with children and young people aged 0-19 with physical and / or learning disabilities and challenging behaviour; you will be part of a multi-disciplinary team delivering services to Nottingham City. The role entails working with children, young people and families within their home environments, including foster care and short breaks placements, to enable independence, safety and access to essential facilities. You will complete individual assessments of need, including specialist postural management; moving and handling and risk assessments leading to the development of multi-faceted care plans. These will include the provision of equipment; minor and major adaptations; postural management seating; moving and handling guidance and equipment; safety interventions and support with rehousing, in line with departmental policy and procedure. ESSENTIAL REQUIREMENTS: The role is exciting and challenging and we are looking for enthusiastic and highly motivated individuals with initiative and a caring nature. Excellent communication skills, planning and organisational skills are essential. A commitment to provide high quality, equitable services to children, young people and families is essential. Previous experience is essential for a level 2 / G grade or level 3 / H grade position.
