The
Texas Health and Human Services Commission (HHSC), in partnership with each
state agency engaged with the Community Resource Coordination Groups (CRCG), submitted the biennial CRCG report to the Governor and 84th Texas Legislature. This report reflects the experiences CRCGs
report in their efforts to provide a coordinated approach to service delivery
for children, youth, adults, and families with multi-agency needs.
CRCGs
voluntarily submit monthly meeting notes, basic demographic data, and other
information to the State CRCG Office, which is housed at HHSC. Based on
information submitted to HHSC by local CRCGs, local CRCGs staffed 750 cases in
calendar year 2012 and 930 cases in calendar year 2013.
Where
active, CRCGs consistently report that the local interagency collaboration
process has improved service delivery for individuals with high needs.

CRCGs are located all
across the State of Texas. Each quarter we will highlight the amazing and
diverse work of Local CRCGs that have come up with innovative practices that
help support the citizens of their individual communities.
This quarter we take a
look at Calhoun County,
the home of Port Lavaca, population 12,000. The Calhoun County CRCG is led by
Dr. Jeannine Griffin, the community's only pediatrician and Karen Kennemer,
Integrated Care Coordinator at Gulf Bend MHMR, the Local Mental Health
Authority (LMHA). The group meets quarterly to share resources and develop a
calendar of events. Cases are staffed on an as-needed basis and different CRCG
members are pre-identified to support families as they engage in the services identified
in the staffing and written on the Individual Service Plan. For example,
families referred to the LMHA for services are supported by Karen who is a
staff member there. The community calendar has helped to build communication and relationships between service providers and community members. Calhoun County CRCG has also developed a list of each
agency partner's services and an identified contact person. This has created a
collaborative inter-agency group that people want to be a part of, demonstrated by participation from the following groups:
- Child Protective Services (CPS)
- Region 3 Education Service Center departments
- Juvenile Probation
- Local independent school district
- Workforce Solutions Golden Crescent
- Department of State Health Services (DSHS)
- Local housing authority
- Department of Aging and Disability Services (DADS)
- Department of Assistive and Rehabilitative Services (DARS)
- United Way of Calhoun County
- Calhoun County YMCA
- Community Ministries Food Pantry
- Wesley Nurses through the Methodist Church

Annual Strengthening Youth and Families Conference
Fedora Galasso from
the State CRCG Office, will be presenting at the 11th Annual
Strengthening Youth and Families Conference October 13-16, 2015 in San Marcos,
TX. There will be a full day pre-conference workshop for CRCG leadership
and members as well as a general CRCG presentation later in the conference for attendees.

When
One Door Closes, Another One Opens
The State CRCG Office is
saying goodbye to interns Max Vanderheyden and Emily Sasser. Max will
continue pursuing his Master's in Education and graduate in 2016. Emily
Sasser has accepted a position with the Texas Home Visiting Program in HHSC.
We wish them luck!
On June 8th we will
welcome two new talented interns, Annabel Osburn, MPA and Lauren Rosales who is
pursuing dual masters degrees in social work and public affairs at the
University of Texas at Austin. We are delighted to have them on our team and
look forward to their support in moving CRCG work forward.

Transformation (ACCEPT) and the Texas Institute for Excellence in Mental Health

The Texas Women's Health Program (TWHP)
TWHP provides family planning services, annual exams and contraception at no cost to eligible, low-income women. Women between the ages of 18-44, not
currently pregnant, uninsured, are U.S. citizens or legal residents and
residents of Texas and make less than the family income limits may access this resource. Visit

Emergency Solutions Grants Program
The Emergency Solutions
Grants Program funds nonprofits and units of government to address homelessness
using a coordinated approach. Applicants should apply with their Continuum of
Care (CoC) and other agencies in one collaborative application to create a
coordinated approach to providing homeless services within the CoC. For program
year 2015, applications were accepted in the spring and awards are expected to
be announced in June 2015.
HOME Investment Partnerships Program
TDHCA's HOME Investment
Partnerships Program (which is offered primarily in rural areas) will now offer
contracts starting with 2015 funds. Previously the HOME Program had offered a
reservation system in which organizations would be able to reserve funds only
after they identified households to be assisted.
Section 811 Project Rental Assistance Program
The Section 811 Project
Rental Assistance Program will accept tenant referrals starting fall of 2015 in
the areas around Dallas/Fort Worth/Arlington, El Paso, Austin/Round Rock, San
Antonio/New Braunfels, McAllen/Edinburg/Mission, Brownsville/Harlingen, and
Houston/The Woodlands/Sugarland. Prospective tenants are people with
disabilities living in institutions, people with serious mental illness, and
youth with disabilities exiting foster care who are receiving services from a
local Section 811 Referral Agent. Referral Agents are DADS, DFPS, and LMHAs.

Department of Aging and Disability Services (DADS)
Aging and Disability Resource Center Grants
DADS has issued small
grants to three Aging and Disability Resource Centers (ADRC) to provide respite care
to family caregivers in emergency or urgent situations that cannot access
respite care through other state programs. The ADRCs are:
ADRC of the Capital Area
(512-916-6045) Serving Bastrop, Blanco, Caldwell, Fayette, Hays, Lee, Travis
and Williamson Counties;
ADRC of Texoma (1-855-937-2372) Serving Bowie, Cass, Cooke, Delta, Fannin, Franklin, Grayson, Hopkins,
Lamar, Morris, Red River and Titus Counties;
West Central ADRC (325-793-8440) Serving Brown, Callahan, Coleman, Comanche, Eastland, Fisher,
Haskell, Jones, Kent, Knox, Mitchell, Nolan, Runnels, Scurry, Shackelford,
Stephens, Stonewall, Taylor and Throckmorton Counties.

of Assistive and Rehabilitative Services (DARS)
Early Childhood
Intervention (ECI)
ECI services is a
statewide program for families with children, birth to 36 months, with
developmental delays or disabilities. Specialists in early intervention help
families and caregivers strengthen their ability to improve their child's
development through everyday activities in the places convenient for the family
such as their home or child care center. ECI offers early intervention services
in every county in Texas. To find the ECI program in your community call the
