Updated Lawful Gambling Forms and Instructions

We have revised the following forms:

• Form G1, Lawful Gambling Monthly Tax Return
• Schedule B2, Report of Barcoded Games
• Barcode Label Sheet
• Form CC, Annual Certified Cash Count by Site
• Form INV, Annual Certified Physical Inventory by Site

Read “What’s New” in the updated instruction booklet for information about these changes.

Begin using these updated forms (marked "Revised 12/15"), for the December 2015 period (due Jan. 20).

Department Closed Monday, Jan. 18

The department will be closed on Monday, Jan. 18, for Martin Luther King Day.
