Partnership4Health
has been partners with Creative Care for Reaching Independence (CCRI) since
April 2013 through their innovative health equity strategy. CCRI provides
services to families and individuals diagnosed with physical and/or mental
disabilities. They have been working closely with their Adult Rehabilitative
Mental Health Services (ARHMS) department. This department revised their client
measurable outcomes with in their Quality Assurance plan to focus on improving
client’s overall health by tracking specific treatment plan goals that focus on
physical activity, weight management or smoking cessation. Their goal was to
have 75% of their clients’ base population achieve that measure. At their last
quarter review, they have achieved 89% of their base population having met the
outcome measures. Kudos to Partnership4Health and CCRI!

To be more effective, public health, health care, and
prevention practitioners must understand the cultural context of their unique
community, and possess the skills and desire to work in those contexts.
Cultural competence, the ability of an individual or organization to interact
effectively with people of different cultures and backgrounds to produce
positive change, is a key component to achieving the Healthy People 2020 goal
of reducing health disparities and achieving health equity. This Progress
Review webinar will highlight the importance of social determinants of health
and cultural competence in working with LGBT and diverse populations to
eliminate health disparities and achieve health equity.

and spatial analysis of geographic regions. Technical assistance they provide
includes a series of workshops on GIS and GIS applications – including the free

The
American Public Health Association (APHA) Physical Activity Section invites
students, educators, researchers, health professionals, and community members
to submit abstracts for the APHA 143rd Annual Meeting to be held October
31-November 4, 2015 in Chicago, IL. They are seeking abstracts that address issues of physical activity and public
health with particular emphasis on research, practice, and policy.
They welcome all submissions related to physical activity and public health;
however, abstracts with outcomes and results are preferred and topics of
particular interest for 2015 include:
- Impacts and outcomes of policies that
promote or discourage physical activity and sedentary behavior in diverse
settings (e.g., schools, communities, worksites, faith-based, etc.).
- Use of policies to address health
inequities and disparities in physical activity participation at all levels of
implementation (local, state, national, etc.).
- Factors influencing the development,
adoption, implementation, and/or acceptance of policies that promote physical
activity or discourage sedentary behavior.
- Perceptions of and adherence to policies
and policy actions among diverse stakeholders (e.g., policymakers, policy
implementers, those affected by policies, etc.).
- Development and/or implementation of
physical activity-related policies that positively impact outcomes other than
physical activity and health (e.g., economic development, environmental
impacts, vehicle traffic calming, etc.).

The revised table of SHIP 3 Monitoring & Evaluation Due
Dates is now posted to the SHIP 3
Evaluation web page. This table was released to grantees via email in
December. Two newly revised Intake & Reach forms have just been posted
online: A revised Intake Form now contains the “Date Partnership Ended” field,
and a revised Schools Reach Form has been repaired so that all checkboxes will
now print. No other changes to Intake & Reach forms were made. Intake &
Reach forms are optional for grantees to use to collect data, and the most
recent versions may always be found on the SHIP web site.
