Survey about your person-centered planning and training experience

DHS and the University of Minnesota's Institute on Community Integration ask people who facilitate and/or train others in person-centered planning to complete the person-centered planning facilitator/trainer survey.

We developed this survey to help us determine the state’s capacity to meet current and future demand for formal person-centered planning and to identify people who can be trainers. Through the survey, we hope to identify people who:

  • Are trained in person-centered planning and what type
  • Are doing person-centered planning and what type
  • Can provide the two-day person-centered thinking training (i.e. “train the trainers”)
  • Can provide person-centered planning training (i.e. “train the trainers”).

The survey takes 15-30 minutes to complete. Please share this message with others who are trained in person-centered planning. We ask that you complete it by March 20, 2016.
