Some people received wrong National Core Indicator survey
Recently, some families and guardians of people with developmental
disabilities were accidently sent the wrong survey for the National Core
Indicators project.
DHS contracts with the national survey firm, Vital Research, to
conduct the National Core Indicator surveys. Some surveys are conducted in
person with people with disabilities, others are mailed to families and
guardians. In the last week of January, Vital Research accidently sent the
wrong survey to some people.
Vital Research currently is calling everyone who received the
incorrect survey to apologize and to offer to conduct the survey over the
phone. At the same time, they are sending an explanation letter (PDF) and
the correct survey to families and guardians.

our contractor, Vital Research. And please encourage people to accept

their experiences.
