Forms Committee Update

The Forms Committee, made of local and county election officials, has been meeting throughout 2015. A lot of progress has been made.

In 2016, BOE will be introducing a brand new Provisional Ballot Form and Envelope.  The committee has taken great care to revise the Four-Step Procedure form to make the process even simpler for election inspectors. And many of the jurisdictions represented by the committee will be piloting the form in their November 2015 election.  All old forms will need to be replaced when the new form is available.

A new mail-in Voter Registration form is in the process of being printed.  The changes should assist voters in completing the form properly. It will be available for ordering in the eLearning Center as well as posted to our website for printing soon.  An announcement will be made in a future News Update when it is available.

Print vendors are in the process of updating the Return (inner) AV envelope.  The minor changes include changing the words "the voter's registration card" to "file" in the Clerk's Use Only box and adding a statement that ballots must be returned to the clerk by 8 p.m. on Election Day.

The current mail-in voter registration form and return (inner) AV envelope may be used until supplies are exhausted.

The Forms Committee continues to review forms and updates will be provided as necessary.
