MDHHS announces upcoming Invitation to Bid for Primary Care Health Homes Program
 

LANSING, Mich. – The Michigan Department of Health and Human Services will soon release an Invitation to Bid for provider participation in the MI Care Team health homes pilot program. The purpose of the MI Care Team program is to improve the health of beneficiaries by integrating primary care services, making it easier on the beneficiary to navigate and receive the physical and behavioral health services they need.

MI Care Team providers will coordinate and manage behavioral and physical health care services for Medicaid and Healthy Michigan Plan beneficiaries who have depression or anxiety and at least one of the following chronic conditions: asthma, diabetes, chronic obstructive pulmonary disease, heart disease, or hypertension. MI Care Team health homes will provide comprehensive care management, care coordination and health promotion, comprehensive transitional care, patient and family support, and referral to community and social support services.

The pilot program is scheduled to begin in April 2016, and is anticipated to run for at least two years. Federally Qualified Health Centers (FQHCs) and Tribal Health Centers (THCs) are eligible to apply to become MI Care Team providers.

Health homes are an optional Medicaid State Plan benefit. Michigan appropriated funding to implement primary care health homes in Michigan’s FQHCs and THCs upon recommendation from the Mental Health and Wellness Commission. This is the first time health homes will be a benefit in Michigan for individuals with mild-to-moderate behavioral health needs.

FQHCs and THCs interested in applying to participate with this program must be registered as vendors in the Buy4Michigan system at www.buy4michigan.com by October 29, 2015 to receive notification of the application’s release. Additional information regarding program expectations will be provided within the Invitation to Bid. All responses with related materials must be submitted electronically using the Buy4Michigan system by December 8, 2015.
