The Live Well Lake County Steering Committee and Lake County
Health Department and Community Health Center are asking Lake County residents
who received an invitation letter regarding a health survey to respond as soon
as possible, so that information can be used to help identify health needs in their
neighborhood. This survey will guide community health improvement plans and
support better health outcomes for all county residents.
During the first week of September, invitation letters were sent
to 5,000 randomly selected households in the county. Questions included in this anonymous survey
address key health priorities including traditional priorities such as chronic
disease. Emerging health topics such as mental health, access to care, and
substance use are also included.
Just one adult (18 years or older) with the most recent birthday from
each selected household is asked to respond to the invitation via an online
form or by calling a toll-free number. Whether using the phone or online
option, respondents should be able to complete the survey in 20 minutes or
less. Results of the survey will be made available on the Health Department’s
website in its Community Health Status Assessment early next year. Any questions
